1. Assumption of Risk & Liability Waiver
By enrolling my child in Odyssey FM Summer Camp, I acknowledge that participation in camp activities involves inherent risks, including but not limited to physical activity, outdoor play, and interaction with other children. I voluntarily assume all risks related to my child’s participation and agree to release and hold harmless Odyssey FM, its staff, volunteers, and affiliates from any claims, liabilities, or expenses arising from injuries, accidents, or other incidents that may occur during camp activities.
I understand that Odyssey FM will take reasonable precautions to ensure safety but cannot guarantee an accident-free environment. In the event of an emergency, camp staff will contact parents or guardians first. If immediate medical attention is required and a parent/guardian cannot be reached, I authorize camp staff to seek medical treatment for my child and assume financial responsibility for any resulting medical expenses.
2. Medical & Health Policy
Parents/guardians must disclose any allergies, medical conditions, or medications their child requires.
Odyssey FM staff cannot administer medication to campers, except in emergency situations with a written medical authorization.
Children experiencing contagious illnesses (fever, flu, vomiting, etc.) must stay home. If a camper shows symptoms of illness during camp, parents will be contacted for immediate pickup.
3. Drop-Off & Pick-Up Policy
Camp hours are 8:30 AM to 4:00 PM.
Drop-off is from 8:30 AM to 9:00 AM. Campers should not be dropped off before this time, as staff will not be available for supervision.
Pick-up is 3:30 PM to 4:00 PM . A late fee of $1 per minute will be charged after 4:10 PM.
4. No Refund Policy
All camp fees are non-refundable, regardless of withdrawal, absence, dismissal, or emergency closures due to weather or other circumstances.
5. Technology & Personal Belongings Policy
Technology should be left at home, unless prior communication has been made with staff.
Odyssey FM is not liable for lost, stolen, or damaged personal items, including but not limited to phones, tablets, and smartwatches.
We recommend keeping toys, trading cards, and expensive items at home to avoid loss or damage.
6. Behavior & Disciplinary Policy
To maintain a safe and respectful environment, campers must follow camp rules and expectations. The following behaviors will result in disciplinary action, including possible removal from camp without refund:
Immediate Grounds for Dismissal (Zero Tolerance Policy):
Physical violence (hitting, kicking, fighting, etc.)
Possession of weapons (knives, guns, or any dangerous items)
Bullying, harassment, or threats toward campers or staff
Possession or use of illegal substances, alcohol, or tobacco
Destruction of camp property or others’ belongings
Progressive Disciplinary Actions for Other Misconduct:
First Offense: Verbal warning and discussion with the camper.
Second Offense: Parent/guardian notification and behavior discussion.
Third Offense: Suspension or removal from camp (without refund) based on the severity of the behavior.
7. Lunch & Snack Policy
Parents are required to pack lunches and snacks for their children each day.
Odyssey FM is a peanut and tree nut-free camp to ensure the safety of children with allergies.
8. Outdoor & Sunscreen Policy
Campers will participate in outdoor activities, including walks to the park.
Parents should provide sunscreen for their child. If necessary, Odyssey FM staff may apply sunscreen to protect campers from sun exposure.
By typing my full name below, I acknowledge that I have read, understand, and agree to the terms and policies outlined in this form, including the liability waiver, refund policy, behavior policy, and emergency procedures.
